Diabetes Self-Management Participant Series

The Diabetes Self-Management Program (DSMP) is an evidence-based program designed by Stanford University to help people who are experiencing diabetes or pre-diabetes. DSMP improves confidence and encourages lifestyle changes to better manage chronic conditions.
Living Healthy workshops meet weekly for 2.5 hours for six weeks. The Living Healthy with Diabetes Self-Management Program is ideal for individuals age 60 and above who are experiencing diabetes or pre-diabetes. Participating in a self-management program can help you learn skills to manage your diabetes more effectively and improve your quality of life.
